What you get:
Organization of all of the medications in the home
We properly sort and dispose of old, expired medications and create a list of what the client is actually taking and when, including all over-the-counter and herbal medications. (may be different than what the physician has prescribed, and creates a baseline in order to have a clear discussion with prescribing doctors).
We contact all of the prescribing doctors and communicate all of the medications that are being taken. Oftentimes it is discovered that the patient is taking too many medications (or the wrong medications) and we come up with a current, up to date list. We bridge the gap that occurs between the doctor’s office and the client’s home.
Ongoing organization of current, up-to-date medications into monthly/weekly/daily pill boxes
We try to fill as many weekly pill boxes as is safe and possible in order to save you money. We often fill enough for 2-6 weeks, depending on the medication, the pharmacy, and the client’s condition.
Extensive teaching /communication with family, case managers, or caregivers
While with your loved one, we ask questions and observe them for possible side effects or unexpected changes. We are available to speak with you anytime and alert you to any changes/concerns as needed.
Updated medication list: We create and provide a current medications list for anytime it is needed (new providers, upcoming procedures, hospital admissions, etc.) Medication lists are updated anytime there is a prescription change.
